NOTICE TO USERS OF THE BOARD FOR ORTHOTIST/PROSTHETIST CERTIFICATION, INC.™ (BOC™) CERTIFICANT AND BOC ACCREDITED FACILITY™ DIRECTORY USE LIMITATIONS POLICY AND DISCLAIMER STATEMENT

The BOC™ Certificant and BOC Accredited Facility™ Directory (Directory) identifies those: individuals who have satisfied the BOC certification standards established by the Board for Orthotist/Prosthetist Certification, Inc.™ (BOC™); and, orthotic and prosthetic service and/or Home Medical Equipment (HME) supply facilities that have satisfied the BOC facility accreditation standards to become a BOC Accredited Facility™. BOC certification standards include requirements related to education, and/or professional experience, as well as, the successful completion of comprehensive knowledge-based examinations. BOC facility accreditation standards include requirements related to orthotic and prosthetic service and/or HME supply facility maintenance, clinical record keeping, and professional insurances.

While BOC makes a reasonable effort to provide current and accurate information in the Directory, BOC cannot and does not warrant or guarantee the accuracy, completeness, timeliness, merchantability, or fitness of the information contained herein for a particular purpose. The identification of any individual practitioner or BOC Accredited Facility in this Directory, in no way constitutes an endorsement or guarantee, either express or implied, of the competency, services, performance, or standards of practice of either the individual BOC certificant, any business, company, corporation, or organization with which the individual may be affiliated, or any orthotic and prosthetic service and/or HME supply facility. While certificants are required to meet established certification and Continuing Professional Education (CPE) requirements, and BOC Accredited Facilities are required to meet established accreditation standards, BOC maintains no control over these certificants, facilities or any related organizations, and disclaims all liability to any party for any action or decision made in reliance on the information contained in the Directory or otherwise provided by BOC, or for any actions or inactions of the certificants and/or BOC Accredited Facility to any party, or for any loss or injury, resulting from the use or non-use of such information.

BOC will provide you with a limited license and permission to use the contents of this Directory if you accept the following Agreement.

AGREEMENT: In order to access and view the Directory, you accept, and agree to be bound by, the following terms and conditions: the information contained in this Directory shall be used for individual, personal, non-commercial, and confidential reference purposes only, and may be used only pursuant to the terms of this license agreement; this Directory and its contents are the sole property of BOC; and the contents of the Directory may not, in whole or in part, be reproduced, copied, disseminated, entered into a computer database, used as part of, or in connection with, a mailing list, or otherwise utilized, in any form or manner, by any means, except for the user’s individual, personal, non-commercial, and confidential reference. If you would like to use the Directory for any other purpose, please contact the Chief Executive Officer to request permission to use the Directory for such purpose. Please be advised that it is within the sole discretion of the Chief Executive Officer to approve your request.
