Reassign CMS DMEPOS Accreditation

Already accredited, but wish you were with BOC? Here's how you can choose BOC as your new accrediting organization.

Are you accredited and in good standing with another accrediting organization (AO)? It's easy to reassign your current accreditation and choose BOC to meet your accreditation needs.

Follow these two easy steps to reassign with BOC:

1. Complete BOC's brief reassignment application.

Fill out your facility's reassignment application. Once you've completed the application, send it to BOC by email at fa@bocusa.org, by fax at 410.581.6228, or by direct mail at 10451 Mill Run Circle, Suite 200, Owings Mills, MD 21117.

2. Along with the application, please send us the following documents:

Verification of good-standing and active accreditation status, such as a copy of your most recent accreditation certificate

A copy of your most recent site survey report

A list of the product categories that are included on your existing accreditation

Copies of your current business/professional licenses and certifications, your most recent Performance Improvement (PI) report, and five current patient files, including the DME item(s) supplies and the corresponding prescription(s)

What Happens Next?

The BOC Accreditation Team will do a comprehensive review of your documentation. If you are unable to provide any of the information listed above, BOC will send one of its objective, consistent site surveyors to your facility. The price of a site survey is $1,199. Soon, we will be proud to call you a BOC-accredited facility. We will send you a new accreditation certificate, featuring the BOC seal, with your facility’s name and accredited product categories.
