Establishes the rules and procedures governing the investigation and resolution of alleged violations of the BOC Accreditation Standards, including the resulting disciplinary measures BOC may apply to businesses found to have violated these standards.

Change/Update Form

Use this application form to request a change in your DMEPOS accreditation due to a change of location, change of ownership and/or adding product codes to your DMEPOS accreditation. Submit and notify BOC at least 60 days before any of the changes go into effect. Each of these changes requires a new unannounced onsite survey.
