You are required to reply to BOC within two weeks (by Tuesday, November 26, 2019) to inform us of your plans.

OR-02 Requirements

I. I am in receipt of BOC's November 12, 2019 letter regarding requirements to maintain accreditation for products in category OR-02.

II. These are my plans.

I would like to voluntarily rescind my accreditation for OR-02, effective immediately. Please send me a new accreditation certificate that omits this category.

I plan to contract with, or hire, an individual with specialized training as defined by Quality Standard Appendix C* to meet product category requirements for OR-02. I will provide you with an update on my plans by November 26, 2019. * Certified Orthotist (CO or BOCO) or Certified Orthotic Fitter (COF or CFo) or a physician, a treating practitioner (a physician assistant, nurse practitioner, or clinical nurse specialist), an occupational therapist, or physical therapist.

I plan to earn the Certified Orthotic Fitter certification myself or encourage an employee to do so. I will provide you with an update on my plans by November 26, 2019. NOTE: As a courtesy, BOC will waive the $150 application fee for suppliers who earn the BOC COF credential. Please see below this form for a special application that reflects this discount.
